Filling Out the Application Form

1st Step

Register to vote at least 29 days before the election in which you hope to vote. The voter registration cutoff starts at 29 days before the election and continues until the election is over, after which you are officially registered to vote for the next election.

​

1. Download and print the Florida Voter Registration Application. If you do not have a computer or printer, you can obtain this form from your nearest public library.

You can also obtain an application at your local Florida DMV. You can do this if you are a new Florida resident or want to update certain information.

​

2. Check "New Registration" in Box 1 unless the following applies. The exception is if you are already registered in Florida and only need to change your name, address, party affiliation, or signature or receive a replacement Voter Information Card.

​

3. Check "Yes" in Box 2 and Boxes 3 and 4 to affirm you are a US citizen with the mental capacity to vote and have never been convicted of murder or sex crimes.

​

4. Write your date of birth in Box 5. Write two digits for the month, two digits for the day in that month, and four digits for the year.

​

5. Write your Florida driver's license or identification card number in Box 6. If you have neither, instead write the last four digits of your Social Security number.

​

6. Write your last, first, and middle name in Box 7. If you are changing your name, write your former name in Box 11.

7. Write your address, city, county, and state (Florida) of legal residence in Box 8. If you use a P.O. Box to receive mail, write its address in Box 9. If you are changing your address on your existing Florida voter registration, write the old address in Box 10.

8. If you wish to vote in primary elections, fill in Box 12 with your party affiliation. You can choose to participate in the primary elections of the Democratic Party, Republican Party, or one of the other political parties registered in Florida.

If you do not declare a party affiliation, you cannot vote in any party's primary elections, as Florida is a "closed primary" state. Under Florida law, you can change your party affiliation until 29 days before an election.

9. Swear or affirm to the oath printed in Box 16 by signing your name in the box and writing the day's date. If you cast a provisional or absentee ballot, the poll worker will compare its signature to the one you provide on the application.

Changing Information if You Are Already Registered to Vote

2nd Step

In Box 1 of the Voter Registration Application, fill out any of the following information you wish to change. Below you will find a list of information you can change, as well as requests you can make for new cards:

​

Notify any address change in Florida. If you move to a different address, you'll need to provide information so that the change can be noted.

If moving to a different address within the same county, contact you Supervisor of Elections and alert them of your new address. You can do this in person, by phone, or by written application, e.g. filling out the Florida Voter Registration Application. When doing so, provide your date of birth.

If moving to a different address outside of the county, fill out the Florida Voter Registration Application with your new address.

Notify any change of party affiliation in Florida. Notify the state of Florida of any change in party affiliation by filling out a voter registration application. This application must include either your date of birth or your voter registration number as proof of identity.

​

Notify any name change in Florida. If you change your name, because of marriage or gender reassignment surgery, for example, notify the state of Florida by filling out a voter registration

application.

​

Notify any change in signature. Since a county official checks the accuracy of the signature in your ballot against the signature on file, having an up-to-date signature is very important.

​

Request a replacement card if you have lost yours.

Sending Out the Application

3rd Step

1. Mail the application to your county Supervisor of Elections. The address for each of Florida's counties is on the back or second page of the application form. Don't forget to place a first-class stamp to the envelope.

  • You may also deliver your application in person to any of the following locations:

    • Any Supervisor of Elections’ office in the state

    • Any driver’s license office

    • Any voter registration agency including an armed forces recruitment office,

    • Any public library

    • The Division of Elections

2. Wait a few weeks for your Voter Information Card to arrive in the mail. It will tell you where you need to go to vote if you do so on Election Day.
